Diabetes Care

The number of people diagnosed with diabetes across the UK is nearly three million, with significant effect on long-term health and quality of life. Type 2 diabetes, which is largely preventable, is continuing to increase in the population.

Care integration can impact on quality of care and improve outcomes at every stage of the patient pathway through prevention, early diagnosis and effective intervention and treatment, which encourages patient self-care and responsibility. Entries for this category can be from individuals or teams responsible for planning or delivering integrated care pathways for this group of patients. Entries will be judged against the general criteria. Submissions could include one or more of the following elements:

  • Health promotion of type 2 risk factors and lifestyle interventions
  • Prompt diagnosis and referral to specialist services
  • Innovative patient education to enable patients to understand their condition and empower them to take responsibility for self-management
  • Effective monitoring of the range of complications of diabetes
  • Intervention to minimise the risk of long-term complications
